Checklist 12B – Registration of Foreign Decree Under Uniform Interstate Family Support Act (UIFSA) – Word

• If filing as a Self-represented Party (without the aid of an attorney) you must print out Checklist 12B along with the required forms. Once the forms are completed, you will need to make 2 copies (original + 2 copies = 3 total) of each required form. All forms must be reviewed by our Compliance Officer before they can be filed.
