CONFIDENTIAL DISCLOSURE OF PERSONAL IDENTIFIERS
Pursuant to Rule 45(D) of the Rules of Superintendence for the Courts of Ohio, parties and their attorneys shall omit personal identifiers, as defined in Rule 44(H) of the Rules of Superintendence for the Courts of Ohio, from all case documents filed with the Clerk of Courts whether filed in paper or electronic form.
Rule 45(D) also authorizes the Clerk of Courts to provide a standard form for parties to use to reference the omitted personal identifiers. This
form is available on this website as well as at the Clerk of Courts office.
